Important Information for New Clients

First, thank you for choosing me as your massage provider.  It is an honor to help you achieve optimum health and vitality through the art of massage.

To prepare for your first visit with me, please review the following tips and information about medical conditions and payment policies.

Also, please download and complete a Client Intake Form, and bring this with you to your first appointment. The information you provide will help me select the best massage modalities and techniques for your current state of health.

Prenatal massage clients, please complete both the Client Intake Form and Physician’s Release Form

Oncology massage clients, please complete both the standard Client Intake Form and Client with Cancer Intake Form

Tips for Being Prepared for Your Visit

You can enhance the effectiveness of your massage treatment by following a few simple tips:

  • Please arrive 5-10 minutes prior to your scheduled appointment. This will allow you time to breathe, use the restroom, drink a glass of water, and talk with me about any particular needs or concerns.
  • Try to leave yourself a little time after your appointment to enjoy the feeling of relaxation and wellness. You may wish to sit in the garden, or take a stroll to the nearby beach.
  • Please turn off your cell phone (or better yet, leave it in your car) during your visit.
  • An intake form must be completed and signed by the client before services are performed.
  • Physician clearance is required for recipients of prenatal massage. You can find the Physician Release Form here.

Disclosure of Medical Conditions

Please note:

  • Massage is provided for the basic purpose of relaxation and relief of muscular tension. The therapist is not qualified to treat, diagnose or prescribe, and nothing your therapist says will be construed as such.
  • Massage is not a replacement for medical care and that certain conditions are contraindicated for massage.
  • Full disclosure of all known medical conditions is required at the time of services. Clients agree to keep the therapist informed as to any changes in their medical profile. There will be no liability on the therapist’s part if this is not done.
  • Any sexually suggestive remarks, innuendoes or advances made will result in immediate termination of the session with the full cost of services charged.

Cancellation and Payment Policies

  1. If you need to reschedule your appointment, please provide at least 24-hour notice.
  2. Appointments cancelled with less than 24-hour notice will be billed at the full service charge. Exceptions may be made for certain circumstances which include: the appointment time can be filled by another; the client is ill or injured, or a death of a loved one has occurred.
  3. Payment must be made in full at the time the service is rendered. Acceptable forms of payment include check, cash, or credit card.
